get pet health insurance to cover those costly vet bills. Well,

Untied States, the pet insurance coverage they've been waiting for.
Pet Insurance is now available to AFT members at surprisingly reasonable
premiums. The plan covers more than 6,400 veterinary medical problems
and conditions for accidents and illnesses. You may also purchase
additional coverage for preventative and routine veterinary care.
Under this plan, pet owners can visit any licensed veterinarian,
veterinary specialist, or animal hospital, anywhere in the world .
Pre-authorization is not required.
What
Is Covered
Many of you will probably be familiar with medical and insurance
terms such as: Wellness Care Coverage; deductibles and co-payments
per visit; limited prescription coverage; pre-existing conditions
and hereditary exclusions; schedule of benefits and other standard
medical language. Pet Insurance helps pet owners pay for their veterinary
care by reimbursing a portion of their costs for:

Is Covered
This plan covers dogs and cats at least six weeks old
(there is no upper age limit.) Your premiums for dogs and cats are
based on the species, age of pet, and the plan type you select.
Birds are insured from the age of 3 months and older and must be
in the owner's possession for a minimum of 60 days. Premiums are
based on the size of the bird. Separate policies are required for
each pet you enroll in the plan, however, discounts are available
for multiple-pet families.

To Use
Pet
Insurance works in a similar fashion to standard indemnity health
insurance for humans. Once your pet has been seen by a veterinarian,
you simply submit your claim form (signed by the veterinarian) along
with the original bill and the diagnosis for any covered procedure.
Your claim will be processed, and you should receive reimbursement
usually within seven days.

Very few treatment exclusions.
Easy-to-understand savings. Each year, we begin reimbursing you 80% of your covered expenses as soon as you spend your $100 annual deductible.
No enrollment fee.
No benefit schedules, which means you will be reimbursed 80% of your veterinarian's total bill (up to your policy's annual limit). Some other pet insurance companies calculate their reimbursement based on a fixed cost per treatment. For instance, if your dog is treated for a ruptured spleen and your veterinarian charges $400 for treatment, Premier will reimburse you 80% of the $400 (if you have already met your $100 deductible that year). But other pet insurance companies
calculate reimbursement based on an amount of their choice. So if your bill is $400, these companies might only reimburse you for 80% of $200, $125 or even $75, depending on their benefit schedule -- you will have to pay for any additional veterinary fees out of your own pocket.

Q: After signing up my pet, can I still see my regular veterinarian?
A: Yes. If your pet is insured with a PetCare Pet Insurance Program you can use the
veterinarian of your choice.
Q: How much are the deductibles?
A: You pay a $50 deductible, unless otherwise noted, per event regardless of the number of
trips you need to make to the veterinarian. This deductible remains constant for as long
as your PetCare Pet Insurance Policy is in force.
Q: How much do I have to pay for any claim I submit?
A: PetCare Pet Insurance Programs pays 100% of payable Claims, less the fixed deductible
on QuickCare and QuickCare Gold programs up to your pet's coverage limit. From the pet's
8th birthday (6th for select breed dogs) the policy holder will be responsible for the
first 20% of any claim.
Q: What are my payment options?
A: Visa, MasterCard, & Discover card are accepted at our call center for Annual or
monthly payments. Currently, Visa and MasterCard are accepted on this Web site.
Check & money order are accepted for annual or quarterly payments.
Q: What if I want to enroll more than one pet?
A: Each pet receives its own policy. As a result, it is necessary to fill out and submit a
separate application for each pet. If you enroll 3 or more pets you may be eligible for a
discount.
Q: How do I make a claim?
A: Mail or fax in a completed one page claim form completed by yourself & your
attending veterinarian along with the detailed receipts.
Q: What is the $10 annual fee?
A: Those who choose to pay their premiums on a monthly or quarterly basis are charged a
$10 fee. No fee applies to those individuals selecting an Annual payment method.
Q: In what states are PetCare Pet Insurance Programs available?
A: Alabama, Arkansas, California, Colorado, Delaware, Florida, Georgia, Idaho,
Illinois, Indiana, Iowa, Kansas, Maryland, Michigan, Mississippi, Missouri, Nebraska,

Q: What are your call center hours?
A: Monday to Friday, 8:00am to 9:00pm Central time; Saturday 10:00am to 5:00pm Central
time. Administrative hours are Monday to Friday, 8:00am to 5:00pm Central time.
Q: How long will it take for my claim to be paid?
A: Our mandate is to process your claim within five business days after receiving your
completed claims form with original receipts attached. If we have any questions about it,
we'll call you immediately.

PetCare Pet Insurance Programs have been designed to meet the
needs of you, the responsible pet owner.
PetCare Insurance Programs provide:
Comprehensive coverage of veterinary fees.
Veterinary fees covered include but are not limited to: prescribed medication, X-rays,
surgeries, hospitalization, ultra-sounds, MRI/CAT scans, homeopathic treatments including
acupuncture and chiropractic, chemotherapy, and referrals. Endeoscopy is also covered in
cases of foreign body ingestion requiring surgery.
Enroll your pet as early as 8 weeks of age.
Coverage for all QuickCare programs can begin as early as 8 weeks of age. For QuickCare
and QuickCare for Indoor Cats, there is no upper age limit for enrollment. For QuickCare
Gold, enrollment is possible up to age 10 for cats, age 8 for dogs (6 for 'select' dog breeds).
Unlimited Accident Coverage.
QuickCare, QuickCare for Indoor Cats and QuickCare Gold policies provide coverage for an
unlimited number of accidents, each covered to the maximum benefit as detailed in the coverage charts
on the appropriate pages. No maximum annual or lifetime accident benefit restrictions.
Flexible illness benefits.
QuickCare for Indoor Cats and QuickCare Gold provide flexible and accessible illness
protection for your pet by covering a wide range of illnesses as detailed in the coverage
chart above. There are no maximum annual illness benefit restrictions, and you choose when
and how you use your illness benefits. The maximum benefit payable for each illness
category is detailed in the coverage charts on the appropriate pages.
Your accident coverage begins immediately.
Accident Coverage begins at 12:01am the morning after your Enrollment application is
approved. Illness Coverage will begin 30 days following Enrollment. No physical
examination is required.
Fast Claims Turn-Around.
Your check will be in the mail within five business days of PetCare Pet Insurance Programs
receiving your completed claim form.
We pay 100% of payable claims.
A PetCare Pet Insurance policy pays 100% of all payable claims, less the fixed deductible
on QuickCare, QuickCare for Indoor Cats and QuickCare Gold programs up to your pet's coverage limit.
Exclusively for QuickCare Gold policy holders, from the pet's 8th birthday (6th for select breed dogs)
the policy holder will be responsible for the first 20% of any claims.
Fixed premiums and deductibles.
Premiums and deductibles do not automatically increase as your pet ages. Premiums are not
higher when you sign up an older pet. You pay a $50 deductible (unless otherwise noted) for each incident
claimed. This deductible remains constant as long as your PetCare Pet Insurance policy is in force.
Guaranteed Acceptance.
All breeds of cats and dogs are accepted into QuickCare, QuickCare for indoor Cats or
QuickCare Gold. For QuickCare Gold, medical records are required at the time of
Enrollment. (Shar Pei breed is eligible for QuickCare coverage only.)
Pick your veterinarian.
With PetCare programs, you can use the veterinarian of your choice.
You may be eligible for an immediate discount.
If you are insuring 3 or more pets, if your pet is registered as a 'Medical Service' pet,
or you are employed at a veterinary clinic, we will give you a 10% discount on each premium.
Our "Total Satisfaction
Guaranteed" clause gives you a chance to reconsider.
From the time you receive your PetCare Pet Insurance policy, you have up to ten (10) days
to change your mind about enrolling. If you have made no claims in that period, we will
void the policy and reimburse your premium upon your written request.
To keep your policy in force, take your
veterinarian's advice.
We ask you to have your pet examined annually by your veterinarian and keep current with
the vaccinations he/she recommends. It's a good way to keep your pet healthy, and it's
also a condition of your continued PetCare Pet Insurance Program coverage.
There is a small "Select" breed
surcharge for dogs covered by QuickCare Gold.
Some breeds are more susceptible to certain illnesses, therefore QuickCare Gold premiums
are slightly higher for the following "Select" breeds: Basenji, Basset Hounds,
Boxers, Bulldogs, Dalmatians, Deerhounds, Doberman Pinschers, Dogue de Bordeaux, German
Shepherds, Greyhounds, Great Danes, Irish Wolfhounds, Leonbergers, all Mastiff Breeds,
Mountain Terriers (Bernese, Estrelia, Pyrenees), Wheaton Terriers, Newfoundlands, Old
English Sheepdogs, Rottweilers, St. Bernards. There are NO "Select" breed
surcharges for QuickCare and QuickCare for Indoor Cats policies exclusively.
A common-sense exclusion to keep premiums
affordable.
We reserve the right to exclude from coverage any foreseeable or existing medical
problems. After we have processed your application, we will send your Document of
Insurance which will indicate any exclusions. For QuickCare Gold, medical records are
required at the time of Enrollment. For QuickCare, medical records are not required. For
QuickCare for Indoor Cats, underwriting will only be done at the time of an illness claim.
Coverage is not available in all states.
Policy renewals.
Your policy is continuous provided your premium payments are up-to-date. Your policy can
be cancelled by you or by us, with proper notice as described in your policy Terms and
Conditions. Claims will only be paid if your premium payments are up-to-date.

Easy
to Use
Once
your pet has been seen by a veterinarian, you simply submit
your claim form (signed by the veterinarian) along with
the original bill and the diagnosis for any covered procedure.
Your
claim will be processed and you should receive reimbursement
usually within seven days.
