I. REFERENCE
Business and Finance Bulletin G-28, "Policy and Regulations Governing
Travel."
II. INTRODUCTION
A. Purpose
This Bulletin outlines the University's Business Travel Accident
Insurance Program for eligible individuals traveling on University
business. The program provides accidental death and dismemberment
insurance coverage for eligible individuals traveling on
official University business. There is also coverage provided for
specified eligible individuals while engaged in certain hazardous
activities.
B. Eligibility for Insurance Coverage
For the specific purpose of the Business Travel Accident Insurance
Program, the following individuals are eligible for coverage:
1. Regents and Regents Designate of the University of California
(except as referred to under Section V.B., below).
2. All employees of the University.
3. Interns, residents, and fellows associated with the University
in connection with emergency medical programs.
4. Professors emeriti traveling on official University business.
5. Faculty on approved sabbatical leave when required to travel as
part of their program of research or study.
Employees at the Los Alamos National Scientific Laboratory are
insured under a separate program administered by the Los Alamos
Laboratory.
III. COVERAGE
An eligible individual will be covered 24 hours a day, worldwide,
against accidental death and dismemberment for up to $100,000
(called the Principal Sum) while traveling on official University
business or while engaged in designated hazardous activities on
behalf of the University (the Principal Sum for Executive Program
members is $250,000).
A. Business Trips
For coverage purposes, an official University business trip
begins when the eligible individual leaves his/her residence or
work site, whichever occurs last, for the purpose of conducting
business away from the work site and ends when he/she returns
to his/her residence or work site, whichever occurs first. If
a business trip exceeds 60 days in length, the eligible
individual will be considered to be located at an alternate
residence and work site and must be on a trip away from the
alternate sites for coverage to apply.
Business trips on behalf of the University will include
personal deviations which occur during and which interrupt the
continuity of an otherwise authorized business trip, but
do not include any loss which occurs during a vacation period
or leave of absence.
Procedures and conditions of travel must be in accordance with
Business and Finance Bulletin G-28 which describes the
University policy and regulations regarding travel.
B. Designated Hazardous Activities
1. Hazardous activities undertaken at the direction of the
University covered by this program are:
a. Transportation of emergency medical patients or donor
organs.
b. Structural inspection.
c. Scuba diving.
d. Seismology and wave studies.
e. Hazardous spills cleanup.
f. Authorized activities of U.C. Police Bomb Unit.
These activities are covered on and off University premises.
2. Coverage while an eligible individual is in command of a
general aviation aircraft will be provided only if the
eligible individual is approved for such travel and in full
compliance with University regulations in accordance with
Business and Finance Bulletin G-28, "Policy and Regulations
Governing Travel."
3. Coverage is provided for war risk hazards outside the United
States in areas designated by the respective insurance
carrier as war risk areas. This provision covers claims
caused or contributed to by war (whether direct or indirect),
hostilities or war-like operations (whether or not war was
declared), invasion, act of foreign enemy, civil war,
rebellion, revolution, insurrection, or military or usurped
power.
4. Coverage is provided for terrorist acts committed in the
Unites States regardless of whether they are also considered
to be acts of war.
5. In compliance with Libyan Sanction Regulations, 31 C.F.R.
Part 550, insurance is not provided in Libya, as this would
violate Section 550.202 of those regulations.
IV. EXCLUSIONS
The business travel policy does not cover any loss, fatal or
non-fatal, caused by or resulting from the following:
A. Suicide, attempted suicide, attempted self-destruction, or
self-destruction while sane or insane.
B. Disease of any kind.
C. Bacterial infections except certain infections which occur
through an accidental cut or wound.
D. Declared or undeclared war or any act thereof in the United
States or outside the scope of the purchased war risk coverage.
E. Service in the military, naval, or air service of any country.
F. Riding as a passenger or otherwise in any vehicle or device for
aerial navigation except:
1. Any civilian aircraft having a current and valid
airworthiness certificate and piloted by an individual who
holds a valid and current certificate of competency or a
rating authorizing that individual to pilot such aircraft.
2. Any transport type aircraft operated by the United States
Armed Forces or similar service of any recognized country.
3. Those aircraft owned or leased by the University which have
been specifically included for coverage.
V. BENEFITS
A. Accidental Death and Dismemberment Indemnity
If injury results in any one of the specific losses shown in the
table below, the insurer will pay, with respect to each insured, the
applicable benefit based on the Principal Sum, provided that not more
than one (the largest) of such benefits will be paid with respect to
injuries resulting from one accident and no benefits will be paid
under more than one of the policy hazards descriptions as a result of
one accident.

"Member" means hand, foot, or eye. "Loss" means, with regard to hand
or foot, actual severance through or above the wrist or ankle joints;
with regard to eye, entire and irrecoverable loss of sight; with
regard to thumb and index finger, actual severance through or above
metacarpophalangeal joints; with regard to speech, entire and
irrecoverable loss of speech; with regard to hearing, entire and
irrecoverable loss of hearing in both ears. "Loss of four fingers of
the same hand" means loss by actual severance through or above the
metacarpophalangeal joints. "Loss of four toes of the
same foot" means loss by actual severance through or above the
metatarsophalangeal joints.
The total amount payable for one accident regardless of the number of
eligible individuals involved is $4,000,000.
B. Permanent Total Disability Benefit
This benefit is available for all individuals eligible for coverage
excluding individual Regents and Regents Designate. The major points
are:
1. Permanent Total Disability is defined as the permanent inability
to engage in each and every occupation for compensation or profit
for which the individual is reasonably qualified by reason of
education, training, or experience.
2. The period of Permanent Total Disability must result from an
injury sustained while on covered University travel or while
engaging in covered extra-hazardous activities.
3. The Permanent Total Disability period must begin within 120 days
of the covered injury.
4. The Permanent Total Disability must continue for a period of
twelve consecutive months before benefits are payable.
5. The benefit level payable is 1 percent of the Principal Sum per
month less any amounts payable under the dismemberment provisions
of the policy.
6. The benefit period is 100 months.
7. If the disabled individual should die during the time disability
benefits are being paid, the designated beneficiary will continue
to receive benefits until the total Principal Sum has been paid.
The cause of death can be other than the original injury.
VI. BENEFICIARY DESIGNATIONS
Under the terms of the insurance policy, benefits payable for loss
of life will be paid automatically to the individual or individuals
(in equal shares) in the order of the following categories of
survivors:
A. Insured's spouse.
B. The insured's child or children (including adopted children).
C. The insured's parents.
D. The insured's brothers and sisters.
E. If none of the above is living, then the insured's estate.
Only those eligible individuals wishing to designate a beneficiary
or beneficiaries different from the order given above should
complete a "Beneficiary Designation" form (see attachment).
Eligible individuals completing a form should be advised that it
remains in effect until it is either changed or revoked. The
beneficiary designation does not automatically end with the return
from a business trip.
The completed "Beneficiary Designation" form should be forwarded to
the Payroll Department for placement in the eligible individual's
file.
VII. SUBMISSION OF CLAIMS
The Office of Risk Management, Office of the President, should be
notified as quickly as possible in the event of a claim under this
program. A claim form will be issued at the time of notification of
loss. However, in order to expedite processing, pertinent data,
including the name(s), department, nature of injury, date of death,
place of accident, and nature of the accident, will be taken by
telephone.

Indiana University's Personal Accident Insurance plan pays
benefits in the event of accidental loss of life or severe injuries which
result in dismemberment.
Full-time, appointed employees of Indiana University can
purchase coverage for themselves, or for themselves and family members.
Various additional benefits are available for such situations
as violent crime, permanent total disability, death of both employee and
spouse in one accident, education or training for surviving children or
spouses.
Eligible employees can purchase coverage of from $30,000
to $500,000.
If family plan coverage is purchased, each family member's
coverage is a percentage of the benefit amount selected.
Travel Assistance Services are available
to plan enrollees.
Eligibility
All full-time appointed employees of Indiana University are
eligible for participation in the Personal Accident Insurance plan; an
enrolled employee may also insure his/her legal spouse and unmarried dependent
children (under age 19, or under age 25 if full-time students).
Plan Contributions
The premiums for coverage under the Personal Accident Insurance
plan are paid in full by the employee.
Enrollment Period
New employees have 60 days from the date of employment to
enroll.
Eligible employees who do not choose to enroll at the time
of employment may do so during the annual Open Enrollment Period.
Changes in plan enrollment may be made in the case of a Qualifying
Event*.
Enrollment continues as long as the employee remains eligible
and pays the premium; coverage for dependent children ends when they no
longer qualify as dependents.
*A Qualifying Event is a significant family or employment
change, such as: marriage, divorce, death of a dependent, birth of a child,
spouse's termination or commencement of employment, and termination or
commencement of leave-without-pay.
Conversion Opportunities
Before age 80, if this group coverage ends for any reason
other than failure to pay premiums, the policy can be converted to an individual
policy without medical certification.
Application for a conversion policy must be made, and the
first premium paid within 31 days after the end of group coverage.

Plan enrollees have access to a customer service center 24
hours a day, 365 days a year, from anywhere in the world (when traveling

Medical Evacuation:
When an unforseen medical emergency requires you
to be evacuated to a medical facility for treatment, AIAS gets you there.
When deemed medically necessary by a AIAS designated physician, AIAS will
arrange and pay for your transportation to the nearest adequate medical
facility that can properly treat your condition.
Repatriation:
In the event that you should pass away while traveling,
AIAS will arrange for all necessary government authorizations, and pay
for the return of your remains to your place of residence for burial or
cremation.
Other Insured Benefits:
. Arrangements for and payment of the safe
return home of any dependent children under the age of 16 if you are hospitalized.
. Arrangements for and payment of a traveling companion's
return or his or her original destination in the event of delays due to
your medical emergency.
. Arrangements for and payment of a visit by a
family member or friend if you are traveling alone and are hospitalized
for at least 10 days.

. Medical referrals for local physicians,
dentists and medical treatment centers in the event of accident or illness
while traveling.
. Prescription assistance to refill a prescription
that has been lost, stolen or depleted.
Assistance with Lost or Stolen Items *
Losing your luggage, passport or other important
documents can be distressing and can cause unnecessary delays. If any of
your belongings are lost or stolen, AIAS will assist you in both locating
and replacing your luggage, documents and any other personal possessions.
Legal Assistance *
If you need legal assistance while traveling, AIAS
can arrange help from local attorneys, embassies and consulates.
Translation & Interpretation Services *
When you need communication assistance in a foreign
country, AIAS will provide telephone translation or local interpreters
for all major languages.
Emergency Travel Services *
. Emergency Cash
When you need cash while traveling, AIAS will advance
up to $250 with your satisfactory guarantee of reimbursement.

In an emergency, AIAS will change or make a new airline,

ELCA TRAVEL/ACCIDENT INSURANCE PLAN ELCA policy for all ELCA staff persons, including Regions and Synods
ELIGIBILITY:
Class I - All Officers and Employees of the Evangelical
Lutheran Church in America (ELCA)
Class II- All non-salaried ELCA Church Council Members, ELCA
Board and Standing Committee Members, all Regional Council Members, all Synod Council
Members, and Other Voluntary Representatives of the ELCA. Self-employed persons receiving
fees for performing consultant services to the ELCA.
Class III - All ELCA Missionaries, their spouses and their
unmarried dependent children under the age of 19 (age 25 if attending an institution of
higher learning).
The following classification of coverage is provided at
an additional charge.
Class IV - All Volunteers and other persons, not included in
any other class, attending ELCA sponsored conventions, conferences, meetings and
activities. This coverage is provided at a charge of .10 cents per person/per day. All
ELCA Divisions, Departments, and Units will be required to file an annual report and
billing will be done at the end of each calendar year.
ALL REGIONS are covered in Class IV for
conventions, conferences, and meetings. The ELCA Office of the
Treasurer - Insurance Management will require an accounting of such events and the number
of attendees at
the end of each calendar year. Coverage for special events such as Youth Gatherings,
Hunger Walks, etc. are
included but the Office of the Treasurer - Insurance Management must be advised so that
the Insurance Carrier is informed of this type of event.
SYNODS may elect Class IV coverage also. This is an
optional coverage in which each synod may choose to participate. Reporting and billing is
done at the end of each calendar year. Coverage for special events such as synod
conventions, youth gatherings, hunger walks, etc. which include large numbers of
attendees/delegates must be reported to the Office of the Treasurer - Insurance Management
so that the Insurance Carrier is advised.
BENEFITS:
Accidental Death & Dismemberment - Accidental Medical
Expense is included in Class IV only.
Principal Sums Applicable:
Class I - 5 times annual Salary/Compensation (less
commissions, overtime or bonus earnings) subject to a maximum benefit amount of
$400,000.
Class II - $50,000
Class III - Missionary - $60,000; Spouse -
$60,000; Dependent Child - $7,500.
Class IV - $50,000 Accidental Death &
Dismemberment
$10,000 Accident Medical Expense - in excess of all other valid and collectible
coverages.
DESCRIPTION OF HAZARDS
Class I, II, and IV: 24 Hour All Risk Insurance
while traveling on ELCA Business away from the premises of the ELCA and while traveling
to, from and during any activity sponsored or supervised by the ELCA.
Class III: Business and Pleasure 24 Hour
All-Risk Insurance.
LIMITS OF LIABILITY:
Felonious Assault - Acts of Violence (See Above
"Principal Sums Applicable")
Medical Evacuation and Repatriation - $5,000.
Aircraft Accidents - $3,000,000 aggregate total per aircraft
accident.
