While we have a low incidence of fire and theft in our housing
facilities, we recommend this insurance vendor if your property is not covered under your
parent's homeowner's policy. This information taken from the N.S.S.I. brochure available

Student Personal Property Plan
National Student Services, Inc. has designed a full service, low
cost program to specifically protect the personal property of students living in residence
halls, fraternity or sorority houses or off-campus apartments or houses. National Student
Services, Inc. has been providing property insurance programs to thousands of students
across the country since 1971.
Theft & Fire - Are You Covered?
What would you do if all your personal possessions computers,
stereo, TV, calculator, typewriter, clothing, books, camera, etc. were stolen or destroyed
by fire? It has happened to many college and university students, and it could happen to
you. It makes sense for you to take stock of what you have and prepare to prevent its
loss.
Losses Covered
We cover all loss or damage occurring to personal property you
own during the policy period. Our coverage applies only to property located within your
residence. If you live in an apartment complex or a dormitory, your residence is defined
as "within your rooms." We also cover personal property you have leased for a
period of at least 90 days. Limited coverage applies to property elsewhere. There is no
coverage for mysterious disappearance (unless stolen), mechanical or electrical breakdown,
wear or tear, corrosion and rust, breakage, earthquake, flood, nuclear war, or
governmental seizure. We pay claims on an Actual Cash Value basis (replacement costs less
depreciation).
What About Coverage Under My Parents' Homeowners Policy?
If you have questions about your parent's homeowners policy, show
this brochure to your agent and ask for advice. Every year hundreds of insurance agents
contact us for enrollment applications for their clients. No homeowner's policy provides
the low deductible "all-risks" type of coverage offered through our Student
Personal Property Plan.
How To Obtain Coverage
To obtain coverage, print out the application form (using your printer and web browser's
printer command) and carefully complete it. Make your check payable to N.S.S.I. and mail
it with your application to:

Your policy will be mailed directly to you within 4 - 8 weeks.
Coverage is effective twenty - four (24) hours after the postmark on your application.
Your cancelled check is proof of coverage. This is a guaranteed no-risk program. When you
receive your policy, take up to thirty (30) days to review it. If for any reason you
change your mind, just return the policy within thirty (30) days and your premium will be
immediately refunded. Cancellations after the thirty day period are subject to a $20
minimum retained premium to cover administrative costs.

Coverage
The basic Student Personal Property Plan supplies
$2,000 of coverage for a twelve (12) month period. You may purchase additional coverage in
units of $500 at a cost of $8 per unit. You may select a $25, $50, $100 deductible. The
deductible amount is simply the amount of loss you agree to absorb in the event you file a
claim. The cost of your insurance policy depends on the amount of coverage you purchase
and the amount of the deductible you select. Choosing a higher deductible lowers the
annual cost of your policy (i.e., $2,000 policy / $25 deductible costs $32 per year;
$2,000 policy / $100 deductible costs $22 per year). The table indicates the cost of the
policy with various deductible options.

within your residence or when property is located in a personally-owned automobile. Theft
loss from your car requires evidence of forced entry. Examples of property covered in the
basic policy:
Clothing: Coats, shoes, jeans, shirts. sweaters, etc.
Linens: Sheets, towels, etc.

Special Provisions
Coverage for all electronic appliances or devices (i.e.,
computer, microwave oven, stereo, etc.) is limited to $200 for any one loss, UNLESS you
list the items on the Scheduled Items form on this application. By placing the items on
the Schedule, you will obtain coverage up to or equal to the amount you declare as the
approximate value.
Coverage for cameras and related equipment is limited to $100 per
loss, UNLESS listed as Scheduled Items. By placing the items on the Schedule, you will
obtain coverage up to or equal to the amount you declare as the approximate value.
Loss or damage to stereo disks, records, albums or cassette tapes
is limited to $100 for any one loss.
Coverage for property located elsewhere on campus in a locked
compartment is limited to $100 per loss. Items in storage OFF-CAMPUS are not covered. We
do not cover property in any storage during the summer.
Coverage for jewelry is limited to 10% of the total policy amount
(i.e. $2,000 policy allows $200 jewelry coverage).
Bicycles are covered only when located within your residence and
only when listed as a Scheduled Item.
The total value of your Scheduled Items must not exceed 75% of the
amount of the policy you intend to purchase (i.e., the basic $2,000 policy allows the
total value of Scheduled Items to be up to $1,500). If your Scheduled Items total is more
than the 75% figure, you may purchase additional insurance coverage in $500 units.
Definition of Loss: Any one incident in which one or more items
of personal property is damaged, destroyed or stolen.

During any international visit, the risk of damage or loss of
personal property or effects is real. It is highly likely that you
are travelling with a number of items that have considerable
value, maybe a personal computer or camera, which the damage to or
loss of may prove to be expensive. Please be aware that this
remains your personal responsibility and that personal property
insurance obtained in your own country before you depart may be
easier and cheaper than trying to sort things out here once you

SECTION 7.1
FOREWORD
7.1(1) Foreword
The Department of Risk Management is responsible for administering risk management and insurance programs in compliance with the Board of Regents, State of Iowa, and Iowa State University's risk management policies and procedures, as well as state and federal laws. The focus of Risk Management is to minimize the risk of financial loss to the University through the identification and analysis of risk, implementation of loss control programs, contractual transfer, or other risk reduction and/or financial techniques. Risk Management administers insurance and self-insurance programs covering property and liability risks, coordinates claims administration including Workers Compensation, and provides consultative services to departments, programs, student organizations, and external events. SECTION 7.2
PROPERTY
7.2(1) Property Insurance - Buildings
General Fund Facilities 2001
The University purchases commercial catastrophic property insurance covering academic/general-funded facilities with a single incident deductible of $2,000,000, for which the State provides payment pursuant to Iowa Code Chapter 29C:20.
The University may seek reimbursement of property losses in excess of $5,000 from the State of Iowa pursuant to Iowa Code Chapter 29C:20. Money from the state contingent fund may be requested for repairing, rebuilding, or restoring state property damaged, destroyed, or lost by fire, storm, theft, or unavoidable cause.
Self-Supporting, Auxiliary Operations & Facilities (2001)
The University maintains conventional property insurance on self-supporting, revenue-producing, and auxiliary facilities that are an integral part of the operations of the University. Insured facilities include the residence halls, the Iowa State Center, the utility system, athletic facilities, and other auxiliary operations.
Property insurance generally covers loss by fire, lightning, smoke, theft, explosion, wind, and hail. Deductibles vary from $1,000 to $100,000.
7.2(2) Property Insurance - Contents
The University Risk Manager is available to assist departments and programs with the identification of risk exposures, the evaluation of risk financing options, and the selection of the most appropriate risk management strategies. Although in many instances retaining risk exposures remains the most cost effective solution, commercial property insurance options are available to departments for items such as computers, scientific and technical equipment, ISU property when off premise, and leased or loaned property to ISU. All commercial insurance purchases are coordinated by Department of Risk Management. Information and assistance are available by calling 294-7711. (2001)
7.2(3) Loss Reporting
All losses or damage to university property by fire, storm, etc., should be reported to Risk Management in the Office of the Vice President for Business and Finance. If Risk Management cannot be immediately contacted, the Department of Public Safety should be notified and they will make the appropriate reports. The University Risk Manager will be responsible for reporting the loss to the proper state offices and should be consulted prior to the replacement of equipment and facilities in order to comply with appropriate sections of the Iowa Code or commercial insurance policy requirements.
7.2(4) Property - Personal and Non-State Owned
Iowa State University does not assume responsibility for personal or non-university owned property kept or stored in university facilities. Individual staff members are responsible for their own belongings, including personal books, journals, library materials, manuscripts, and other personal belongings that may be kept on the campus. Should damage or loss occur, the payment for loss or replacement would be the responsibility of the individual. Staff members are encouraged to be sure that their personal insurance covers their personal property kept away from home. Generally, this is a standard provision of most personal insurance policies, although specific riders may be appropriate for staff members who engage in consulting services or have valuable professional items or collections kept on campus. In the case of manuscripts, computer data, or other materials, staff members are encouraged to maintain duplicate or back-up copies at another site to provide protection against loss. (2001)
In cases where the university has been negligent or assumes the responsibility because the materials have been specifically authorized to be utilized in a university event, appropriate insurance should be obtained. Subject to central administrative approval, reimbursement for damage or losses may be made for up to $4,000. However, this is only done on rare occasions and will require both the recommendation from the department and central administrative approval, including the availability of funds from the operating unit. The State Tort Claims Act has provisions for filing claims with the State of Iowa for damage where it is possible to establish that the state has been negligent. These claims are processed by the State Appeal Board and involve extensive documentation and review before approvals are made. Individuals desiring further information should contact the Department of Risk Management in the Office of the Vice President for Business and Finance. (2001)
SECTION 7.3
LIABILITY
7.3(1) Iowa Code, Chapter 669 (2001)
Iowa State University is an agency of the State of Iowa and as such, is self-insured for liability. Claims against the State of Iowa are handled as provided in the Iowa Tort Claims Act (Iowa Code, Chapter 669), that also sets forth the procedures by which tort claims may be brought.
Under Chapter 669, claims may be filed against the State on account of wrongful death, personal injury, or property damage (including reasonable attorney fees) incurred by reason of the negligence of the University or its employees while acting within the scope of employment.
The State will defend, indemnify, and hold the University and its employees harmless against any and all tort claims under the U.S. Constitution, statutes or rules of the United State and/or any other state, but will not cover willful or wanton acts, omissions, or malfeasance in office.
Definition of Employee:
The above provisions apply to all employees of Iowa State University including faculty, staff, and graduate assistants on appointment, or any other individual full or part time, including students, volunteers, and agents acting in a temporary or permanent capacity on behalf of the institution.
7.3(2) Commercial Liability Insurance (2001)
In general, Iowa State University does not purchase commercial liability insurance. Instead, the University relies on certain statutory protections for the University or its employees, pursuant to the Code of Iowa, Chapter 669 (see above).
The University may purchase liability insurance or participate in self-insured liability pooling or other arrangements for professional liability, motor vehicle liability, or other liability if required by statute, contract, or special circumstance. Refer questions, requests for insurance, or certification regarding liability insurance and self-insurance programs to the Department of Risk Management.
7.3(3) Claim Procedures
The provisions of the Tort Claims Act permit claims to be filed with the State Appeal Board, which is composed of the State Auditor, the Treasurer of the State, and the State Comptroller. The State Attorney General acts as legal counsel for the appeal board and generally investigates and recommends procedures for settling individual claims. Under this act, there are no limits or restrictions on the amount of a claim that can be filed. Specific forms for filing claims with the state have been developed and are available from the Department of Risk Management by calling 4-7711. These forms are self-explanatory and indicate the procedures to be followed in filing a claim under the Iowa Tort Claims Act.
SECTION 7.4
MOTOR VEHICLE COVERAGE
7.4(1) University Vehicles
Primary Liability (University Vehicles)
Iowa State University participates with other Board of Regents, State of Iowa institutions in the Regents Motor Vehicle Self-Insurance Program for liability on motor vehicles. The program responds to liability claims for property damage or bodily injury arising from an accident involving University vehicles where the University driver was more than 50% negligent. Claims are processed pursuant to the Iowa Tort Claims Act, Chapter 669 of the Code of Iowa. (2001)
Physical Damage Coverage
The Regents Motor Vehicle Self-Insurance Program includes coverage for comprehensive and collision damage to University vehicles, subject to a $250 deductible per loss. The deductible will be assessed to the department. (2001)
Vehicle Accident Reporting
University drivers are required to report any accident or incident of damage related to the use or operation of University vehicles within 48 hours. A personal injury accident should be reported immediately. Accidents involving University vehicles should be reported to Transportation Services. "Hit and run" and vandalism incidents are criminal incidents and should be reported within 24 hours to the Department of Public Safety or police, if off campus. (2001)
The driver must complete a Regents Vehicle Accident Report form and attach a copy of the police report and completed State Report (SR.21 or SR.22) if one is required. A State Report is required when the total combined damage to the vehicles is $1,000 or more, or a personal injury is involved. Completed reports should be forwarded to Transportation Services within 48 hours of the incident. (2001)
All Regents institutions' vehicle accidents are investigated and adjusted through the State Accident Advisor/Risk Manager, State of Iowa, Department of General Services (Vehicle Dispatch). (2001)
7.4(2) Other Motor Vehicles
Excess Liability (Non-Owned, Borrowed, Personal Vehicles)
For non-owned (including rentals, personal, or borrowed) vehicles, the Regents Motor Vehicle Self-Insurance Program will apply only as an excess liability coverage. The University's travel reimbursement program includes a provision for insurance expenses in the mileage rate calculation for personal vehicle use. In an accident, the non-owned vehicle owner must look to his or her own automobile insurance coverage as the primary insurance. (2001)
The University's excess liability coverage will apply when the driver was an authorized University employee on University business at the time of the loss, and the vehicle owner's policy has been exhausted. (2001)
The University will not reimburse an employee, or other private owner, or their insurer for any deductible or liability claim paid by their auto insurance policy, or for any damage to the vehicle. (2001)
In the event you are involved in an accident in a non-owned, private, or borrowed vehicle while you are conducting University business, report the accident immediately to the Department of Risk Management, Office of the Vice President for Business and Finance, 125 Beardshear, 294-7711. (2001)
7.4(3) Aircraft
All university-owned and operated aircraft are covered by bodily injury (including passenger) and property damage insurance of $10,000,000 for each occurrence.
7.4(4) Student Travel - Insurance
Staff members using their own automobiles to transport students on field trips, inspection trips, etc. are required to carry bodily injury liability insurance of at least the minimum required by the State of Iowa. It is not necessary that students purchase travel insurance for university-sponsored events. However, any unusual program should be discussed with University Risk Management in the Office of the Vice President for Business and Finance to determine if any special arrangements are necessary.
SECTION 7.5
WORKERS COMPENSATION/EMPLOYEE INJURIES AT WORK (2001)
7.5(1) Workers Compensation/Employee Injuries at Work (2001)
Risk Management is responsible for the coordination of the Workers Compensation Program for Iowa State University. Risk Management is located in the Office of the Vice President for Business and Finance, Room 125, Beardshear Hall (294-7711) and is the university contact for questions. Information concerning workers compensation such as how to submit a claim, approved medical providers, or employee return to work is also available on the Risk Management web site.
7.5(2) What is Workers Compensation?
Workers Compensation is a part of the Iowa Code designed to provide certain benefits to employees who sustain injuries, occupational illnesses or occupational hearing loss in the course of and arising out of their employment. Benefits are administered according to Iowa law.
Employees should notify their departmental supervisor, who will call Iowa State University's approved medical provider to schedule an appointment unless emergency care is required. Employees who choose to be treated by any other treatment center and/or physician may not qualify for any workers compensation insurance benefits and may also be responsible for all medical costs related to this incident. This is in accordance with workers compensation benefits for state employees.
An approved list of medical centers for consultation or treatment of work related injuries and illnesses is available by calling Risk Management at 294-7711.
7.5(3) Determining if an Accident or Injury Should be Reported
All accidents and injuries occurring at work or in the course of employment must be reported to the employee's supervisor, even if no medical attention is required. The supervisor is responsible for completing a First Report of Injury Form and submitting it to Risk Management within 24 hours. Incidents that are not reported may cause employees to be ineligible for future benefits related to this injury or illness.
An injury or illness is considered work-related for OSHA reporting purposes if an event or exposure in the work environment caused or contributed to a condition or significantly aggravated a pre-existing condition. This include injuries while the employee is on break, in the restroom, traveling on business or engaged in work activities such as training required by the employer.
7.5(4) Benefit Description
Workers Compensation benefits include payment of medical bills and payment of lost wages beginning with the fourth day off work.
An employee cannot receive reimbursement for medical expenses from both Workers Compensation and a group medical plan. Workers Compensation does not reimburse employees for clothing. Expense for eyeglasses will be covered if it is a result of a legitimate work-related accident.
Payment of lost wages is made on a prorated basis. Employees can elect to:
Receive only Workers Compensation benefits; or
To supplement these payments with their accrued sick leave, vacation, converted sick leave, or compensatory time and remain at full pay.
Questions regarding employee election forms may be directed to the Claims Administrator in the Department of Risk Management, Room 125 Beardshear Hall.
7.5(5) Employee Responsibility
It is the responsibility of the employee to notify his/her supervisor immediately of all work-related injuries, illnesses, or hearing loss. Employees receiving medical treatment must notify his/her supervisor of any work restrictions recommended by the medical provider and provide the department with a copy of Work Status Reports signed by the attending physician.
7.5(6) Departmental Responsibilities
Employees seeking treatment for work related injuries or illnesses should notify their departmental supervisor unless emergency treatment is required. Hospitalization or death of an employee must be reported to Risk Management at 294-7711 within 8 hours, as required by OSHA.
Supervisors should attend to the injured and then determine what condition(s) or act(s) caused the injury or illness. The department is responsible for submitting the First Report of Injury Form necessary to document a work related accident, injury, illness, or hearing loss. It is also the departmental responsibility to notify Risk Management of any changes in the claim status; i.e. lost work time, return to work restrictions, inability to accommodate restrictions, etc. Copies of all Work Status Reports provided to the department should also be forwarded to Risk Management.
7.5(7) Follow-Up Care - Time Lost for Medical Appointments
When an employee who has met the three-day waiting period to qualify for workers compensation returns to work on either a full time basis or full time with restrictions, any subsequent work time lost for medical services will be paid at the regular rate of pay. The employee's accrued leave should not be charged for these leaves, nor are they payable by workers compensation.

All IPFW students who are currently enrolled in
six or more credit hours may choose to purchase health insurance sponsored
by Indiana University. Subscribers may also elect coverage for spouses
and dependent children. Information and applications are available

Property Insurance
Student personal property insurance is also available
to all IPFW students. This type of renter's insurance is offered only
to students through National Student Services, Inc. This plan typically
covers the loss or damage of your personal property due to fire, theft,
or vandalism while in your apartment or locked automobile. Each policy
carries up to $10,000 in coverage Call 1-800-654-6814 for further

Property
When departmental property becomes damaged or lost, a Departmental
Property Damage/Theft Claim Form ( interactive
PDF ) should be completed by a contact person within the department
who will be handling the claim. The contact person should also notify
Risk Management before submitting the claim in case necessary arrangements
have to be made to view the scene of the damage/theft to the damaged
property. The Office of Risk Management will then review the claim
and determine after investigation if the department is to be compensated
for the loss. Departmental property claims are adjusted within the
coverage guidelines of the University's property insurance policy.
Among the notable exclusions: No coverage for mechanical breakdown,
wear & tear, nor any theft unless there are visible signs of
a forced entry. If the claim is accepted, the department will be
notified and asked to provide receipts and/or invoices for all labor
and materials. The department will also need to pay a $250 deductible
which will be subtracted from the total amount reimbursed to the
department via journal entry. Before a department is reimbursed
for their damages, the replacement items must be purchased or repair
work must be completed.
Please note: This policy applies only to University owned or leased
equipment. Coverage is not provided for personally owned
items of employees. This includes shipments or deliveries of personal
items to employees at the workplace. Any employee receiving personal
property at the workplace does so at their own risk. Boston University
acts as a receiver only for University owned or leased property,
and will not be responsible for any lost/damaged personal items
shipped or delivered to employees at the workplace.
Student
Property
Boston University does not insure the personal property of its
students and will only consider reimbursement in those instances
in which the University is responsible for the damage. No theft
claims will be reimbursed. When a student's property becomes damaged,
and the student feels that the University is responsible, the student
should notify a Residence Life Senior Staff member. The student
should also fill out a Student Property
Damage/Loss Claim Form ( interactive
PDF ) describing in detail how the damage or loss came about
and list the items which need to be replaced. Once the claim form
has been filled out, the student should have the Resident Assistant
sign off on it and then the Resident Assistant should send the claim
form to the Office of Risk Management. The Office of Risk Management
will then review the claim and determine after investigation if
the student is to be compensated for the loss. The student should
not throw out damaged property unless authorized by the Resident
Assistant or the Office of Risk Management. If the claim is accepted,
the student will be notified and will be able to pick up his/her
reimbursement check after he/she signs a release form and Risk Management
takes possession of the property.
