Dental Coverage
We represent a dental referral organization that offers membership in its discount dental plan. Members can save 25% to 65% on all restorative and cosmetic work (fillings, crowns, braces, etc.) and up to 80% on preventative work (teeth cleaning, x-rays, etc.) General dentistry and all specialties where available are covered, but you must see dental providers that accept the plan.
Since the dental plan is not insurance or a health care organization, all pre-existing conditions are covered immediately, except orthodontic treatment already in progress. There is no waiting period before you can start using the plan. There are no deductibles, no claim forms and no limits on visits to your dentist.
The monthly cost for this dental plan is $11.95 for an individual, and $19.95 to cover everybody that lives in your household. This includes spouses and children, roommates, housekeepers, even "significant others" if they live with you. You can pay monthly or quarterly with their automatic bank debit, and monthly, quarterly or annually with a credit card. There is a one-time registration fee of $20.
Included with your dental plan membership are also vision benefits, prescription drug benefits and chiropractic benefits. With the vision benefit, you'll save from 20% to 60% on all frames, lenses, tints, etc. at most national chains like Pearle Vision, Sears, JCPenney and Montgomery Ward. You can save from 5% to 30% on opthalmology eye exams and surgery (including RK and ALK) with certain providers.
With the prescription drug benefit, you'll save from 5% to 25% on MOST brand name prescriptions, and 5% to 50% on MOST generic prescriptions and ALL prescription drugs are covered. Over 50,000 retail pharmacy locations nationwide accept this plan, including most national chains, like Osco, Target, Kmart and Walmart, Rite Aid and Revco. With the chiropractic benefits, you save 30% to 50% at over 6,000 chiropractors across the country, with no limit on the number of visits.

Group Insurance Plans
All full-time employees are eligible for our group medical, dental, life and disability insurance. Temporary, and part-time employees, are not eligible under the Dharma plan. Eligibility for these benefit plans begins on the 1st of the month coinciding with or following the date of hire.
Medical Insurance - Anthem BlueCross BlueShield
The Anthem BlueCross BlueShield medical plan gives you access to benefits for care from a network of credentialed providers, plus the freedom to seek care outside this network.
When you work through your primary physician and visit network providers, you receive virtually 100% coverage after making a small physician co-pay. If, on the other hand, you do not go through your primary physician or you choose non-network providers, you pay more towards the cost of your care in most cases.
Dental Insurance - MetLife
MetLife's Preferred Dentist Program (PDP) provides an opportunity to save money by helping you make the most of your dental benefits plan. The plan emphasizes preventive care by paying 100% of the reasonable and customary cost for routine cleanings and exams for you and your family. It also pays a substantial percentage of most other dental expenses to an annual maximum benefit of $1,000 per year.
Enrolling Your Dependents in Medical and Dental Insurance
If you are an eligible full-time employee, you may enroll your eligible dependents in the Dharma medical and dental plans. Eligible dependents include your spouse as well as unmarried children under the age of 19. Children over the age of 19 who are full-time students may be enrolled in the plan to the age of 25, provided that they are attending school regularly and depend on you for support. Other eligible dependents are dependent, unmarried children of any age who are incapable of self-support due to mental retardation or a physical disability which began prior to reaching the limiting age, provided premium payments are continued.
The Dharma Group Insurance Plan is an unbundled plan, meaning that you and your dependents can choose the coverage (medical and/or dental). As an Dharma employee, you are responsible for partial payment of the premium. You can make your contributions with pre-tax dollars through the Dharma pre-tax premium plan. This means your contributions are deducted from your pay before federal, state and FICA taxes are taken out, thus reducing the net impact of these costs on your paycheck. Please consult the Human Resources Department for more information on the premiums for enrollment.
Life Insurance - PHOENIX
Group Life Insurance coverage offers basic protection to all full-time employees. The amount of life insurance you have is equal to $50,000, payable to your beneficiary in the event of your death. If you should die as the result of an accident, the benefit amount will be doubled (i.e. equal to $100,000). Your Life Insurance coverage becomes effective on the 1st of the month coinciding with or following the date of hire.

Short-Term Disability Insurance - PHOENIX
After 15 calendar days of absence due to illness or injury you will be eligible for short-term disability benefits. You will receive 60% of pay up to $1,000 per week. This benefit will provide you with a source of income during extended absences beginning on the 15th calendar day and extending through the 90th calendar day of disability. Amounts paid by Dharma's short term disability plan will be off-set by any state disability and/or worker's compensation benefits you are entitled to receive.

Long-Term Disability Insurance - PHOENIX
Long-Term Disability Insurance coverage is provided for all full-time employees. This coverage provides a partial continuation of salary should a disabling event cause you to be unable to work. The Long-Term Disability plan provides you with payment above that paid by your State Disability Insurance or Workers' Compensation Insurance, to bring your total disability benefits to 60% of your pre-disability salary, subject to certain maximum limits outlined in the plan documentation.
In order to be eligible for Long-Term Disability Insurance payments, you must be disabled for a period of 90 (ninety) days.

Dharma Retirement Plan - Fidelity
The Fidelity SARSEP (Salary Reduction Simplified Employee Pension) enables you to save for your own retirement by deferring a portion of your salary on a pre-tax basis to a SEP-IRA. All SARSEP contributions and any earnings grow tax-deferred until withdrawn at retirement. You may defer up to the lesser of 15% of compensation or $10,000 (maximum for 1999 tax year).
Fidelity SEP-IRA offers a broad array of investment options including a wide variety of Fidelity mutual funds - including money markets, bond funds, and stock funds. Not only does Fidelity offer you investment choice, they also offer you excellent value for the money.

Dharma Stock Option Plan
Some Employees may be granted Incentive Stock Options upon their hire or during their tenure with Dharma Systems. Stock options generally vest over a period of four years. Stock option grants must be approved by the Board of Directors, and the price of your stock option is equal to the fair market value of the shares of Dharma's common stock on the date granted. Consult your Stock Options agreement for details regarding your stock option grant.
