Help With Medicare Expenses Some Very Important Things To Know When Applying You are NOT required to have an in person interview A case worker at your County Department of Job and Family Services can help you if you are having problems getting the needed information. It is important that you inform the caseworker if you need help. Make sure that you fill in the amount of your monthly income and list your assets on your application. You will need to include some type of proof of your assets (this includes recent bank statements, insurance policies, and titles.   Do not send original documents in the mail - Send copies! You should mail your completed application and documents to the County Department of Job and Family Services in the county where you live.   Click here to get addresses of County Departments of Job and Family Services. You can also call the Ohio Consumer Hotline at 1-800-324-8680 to find out where you should mail your application. These four forms are part of the application process: Adobe Acrobat Reader 4.0 or higher is required to view PDF files on this site. The software is free and may be downloaded from the Adobe website . If you are using Adobe Acrobat Reader 3.0, some PDF files may display incorrectly. The Application Information Letter - identifies and explains all programs you may be able to apply for on the "Application for Help With Medicare Expenses." Explanation of Benefits - This form gives detailed information as to who is eligible, income guidelines, and the benefits you can receive. "Application for Help With Medicare Expenses" - This application is a four page form and it must be completely filled out, signed, dated and mailed to your local County Department of Job and Family Services. Please don't forget to send a copy of your documentation. The application is easy to fill out. Simply fill in the information by typing using your PC keyboard. "Your Rights and Responsibilities as a Consumer of Medicaid Health Coverage" - This form must also be signed and dated and returned with your "Help With Medicare Expenses"application. Please read this form carefully and make sure that you understand all of the information. You should print, sign, and keep a second copy for your records. Some Things That You Must Send In As Proof Include: Proof of age: Examples of this type of proof include: A birth certificate, baptismal record, hospital birth verification, notarized statements. You will need to provide a copy of proof for each member of you family for whom you are applying for Assistance Proof of your Income: Pay stubs for the previous month, or most recent four week period; Sources of income also include: Social Security, SSI, VA benefits, alimony. Proof of resources or assets: List any resources or assets that you or your spouse may own. This includes: Savings accounts, checking accounts, stocks/bonds, automobiles, life insurance, trust funds. Social Security numbers: You will need to provide copies of social security cards, or proof that you have applied for a Social Security number for everyone for whom you are applying for assistance. Other Health Insurance: If you or your children have medical coverage through any other health insurance plan, you will need to send in a copy of your card or other proof of coverage.     Return to Top
