22,000 Americans over the age of 50 every two years. Supported by the
National Institute on Aging, the study paints an emerging portrait of an
aging America's physical and mental health, insurance coverage, financial
status, family support systems, labor market status, and retirement
planning.

Why Students Need Health Insurance
University Health Service can serve
many of your routine health care needs. Still, we encourage you
to carry health insurance in case you need additional services.
If you do not have health insurance, you will be responsible
for medical costs incurred that are not covered by the health
service fee, including charges such as emergency room care, prescription
medications, ambulance services, hospitalization and fees from
other health care facilities.
An Important Suggestion: If you do not have insurance, consider purchasing
the MSA Student Accident and Health
Insurance Plan .
International students are required
to purchase insurance that meets University standards, such as
the UM International Student/Scholar
Health Insurance Plan . International
students with other health insurance must check with the International Center ,
Health Insurance Office at (734) 647-2303
or international-insurance@umich.edu to make sure the policy
meets University standards.
Insurance Plans for Students
MSA Student Accident and Health Insurance Plan
(The Chickering Group)
This summary is intended to provide
highlights of the plan. A detailed
description of benefits, limitations and exclusions can be found
at the Chickering
website or in the MSA Student Accident
and Health Insurance brochure, available at the UHS Managed Care/Student
Insurance Office (room 2109) or the Campus Information Center
in the Michigan Union.
Eligibility This
plan is available to:
All students who are currently enrolled,
between semesters or have maintained at least 6 credit hours
during the school year
Graduate and doctoral students who
are completing graduation requirements but not taking courses
Students with dual citizenship
Visiting scholars
International students
UM students enrolled at Flint or Dearborn
campuses
Eligible
dependents, spouses, domestic partners (for details, contact
the Managed Care/Student Insurance office at 734-764-5182).
Basic Review of Coverage
MSA Student Insurance is a health plan administered by the
Chickering Group. You should see a clinician at University
Health Service to manage your medical care and to avoid any deductibles
for referrals. Emergency care fees may be covered anywhere. Non-emergency
medical care may be covered. For details, check with the Chickering Group .
The provider network for the Ann Arbor campus includes:

Outpatient services such as office visits,
lab tests, x-rays are covered you are sick, injured or pregnant,
up to $250,000 maximum benefit. There is a $50 per illness deductible
which must be met before your medical coverage begins, unless
you are treated at or have a referral from UHS. The deductible
will be reduced to $10 for treatment received or referred by
the East Ann Arbor Health Center or the Nurse Managed Centers.
There will be a $10 charge per illness or injury when treating
a student not enrolled for the current semester and for a spouse,
partner or dependent.
In-patient hospital services such as admissions, physician services,
and surgery may be covered by the MSA Student Insurance Plan
after pre-admission certification.
There are some mental health and substance abuse services provided
under this plan. Currently enrolled students may use the UM Counseling and Psychological
Services (CAPS) .
Prescription drugs are covered at participating pharmacies with
co-pays of $7 for generic drugs and $14 for brand name drugs.
Prescription drugs are covered up to $1500 per policy year. In
addition, there is $200 per year coverage allotted for prescription
contraceptives.
There is a vision benefit included in this plan.
This Plan Does Not Cover:
Dental services
Pre-existing conditions, including
pregnancy (will not be covered until a 180 day waiting period
has been met)
Preventive care
How MSA Student Insurance Works with
University Health Service
MSA Student Insurance pays for covered medical services received
at UHS which are not covered by the health service fee. For non-enrolled
students and eligible dependents, services are billable to MSA
insurance on a fee-for-service basis.

Eligibility
All international students, scholars and their dependents are
eligible for and required to enroll in this plan unless they
have health insurance that meets the University's minimum standards.
U.S. citizens and permanent residents (green card holders) are
not eligible for this plan.
Basic Review of Coverage
International Insurance is a health plan administered by the
Chickering Group. It provides hospitalization, emergency room
care and physician visits. You may receive non-emergency medical
or emergency care anywhere.
For maximum cost savings, it is recommended
that you see a clinician at one of the following locations to
manage your medical care:
University Health Service
East Ann Arbor Health Center (734)

Nurse Managed Centers located on North
Maple (734) 998-6117 and North Campus (734) 647-1636
There is a $50 per illness deductible
which must be met before the plan will cover expenses. There
is a maximum of $250,000 for each illness, injury or pregnancy.
Outpatient services (such as office visits, lab tests, and x-rays)
are covered if you are sick, injured or pregnant. The $50 deductible
is waived if you are treated at or referred by UHS. For pediatric
care, go to either Nurse Managed Center on North Campus or East
Ann Arbor Health Center where there is a $10 deductible per injury
or illness.

pre-admission certification. There are some mental health and

physical exams and immunizations are covered for children up
to age 3)
This Plan Has Limited Coverage for:
Pre-existing conditions, including
pregnancy, until covered by the plan for 6 continuous months
[ Pre-existing Condition Limitation: Covered medical expenses
are payable on the same basis as any other condition up to a
maximum of $1,000 per accident or sickness for the covered persons.
This limitation will no longer apply when the covered person
has been continuously insured for 6 consecutive months. This
benefit is subject to all other policy conditions and limitations.

GradCare
This summary is intended to provide
highlights of the plan. A detailed
description of benefits, limitations and exclusions can be found
in the GradCare Member handbook, available from M-CARE Customer
Service by calling (734) 913-2211.
Eligibility :
GradCare is available to Graduate
Student Instructors (GSIs) or Graduate Student Research Assistants
(GSRAs) with a 25% or greater appointment and at least a 4 month
appointment during Fall and Winter semesters or a 2 month appointment
during Spring or Summer. GSRAs must have an appointment of 4
consecutive months at all times.
GradCare is also available to s tudents in sponsored groups. A sponsored group
assures students that full monthly University contribution is
available to sponsor students' health care and allows the addition
of new dependents to the sponsored group. These groups include,
but are not limited to:
-- The holder of a designated Rackham fellowships
-- An eligible LSA Regents Fellow
-- A graduate student or NSF Fellow in a designated department
Check with your academic department
if you are unsure whether you qualify. You may also enroll your
eligible dependents.
Note: Graduate Student Teaching Assistants
are eligible for other UM insurance options.
Basic Review of Coverage:
GradCare is a medical plan administered by M-CARE, Inc. which
provides hospitalization, emergency and non-emergency care. You
must choose a primary care physician (PCP) to manage all of your
medical care. GradCare provides worldwide care for medical emergency
or accidental injury. Non-emergency medical care may be covered
with non-M-Care providers only when the member has obtained M-CARE
authorization for services.
The provider network in Ann Arbor includes: UHS, University of
Michigan Health System and participating community pediatricians.
There are no deductibles to meet before the plan begins to cover
expenses. Outpatient services (such as office visits, mental
health, chemical dependency, routine physical exams, routine
pediatric exams, routine immunizations, allergy testing and injections)
are covered with a co-pay. In-patient hospital services (such
as admissions, physician services and surgery) are covered in
full.
Prescription drugs are covered with a co-pay at any pharmacy
that participates with PCS insurance, including the UHS Pharmacy.
One vision exam per year is covered
up to $40 by UHS and other plan-affiliated optometrists.
How GradCare works with University Health Service
When UHS is used as your primary care provider, the $10 office
visit co-pay is waived for currently enrolled students. Parking
in the Fletcher Street Parking Structure is available at no charge
for GradCare clients.

Other Insurance
Many students are covered by other private
health insurance plans outside the University (for example, through
a parent's insurance plan). Note: International students are required to purchase
insurance that meets University standards. International students
with other health insurance must check with the International
Center , Health Insurance Office at
(734) 647-2303 or international-insurance@umich.edu to make sure
the policy meets University standards.
Important Suggestions:
Contact your insurance company to learn
about your coverage. If your plan is an HMO or PPO, your coverage
may require a referral.
Get your own health insurance card
(or a copy of both sides) and carry it with you along with the
following insurance information:
-- name of the insurance company
-- address and phone number of the
insurance company
-- name of the primary subscriber to
the policy
-- policy numbers
-- contract number
How Private Health Insurance Works
with University Health Service
We strongly encourage students
to be covered under a private health insurance plan . However, this is not a requirement to receive
care at UHS. Many UHS services are covered by the health service
fee (when a student is enrolled for the current semester on the
Ann Arbor campus). Private health insurance is recommended in
order to cover medical care received outside UHS (for
example, emergency room visits, hospitalizations or ambulance
service). These are not covered by the health service
fee. Charges for these services are the patient's responsibility.
UHS will bill insurance companies for
certain procedures and diagnostic tests performed at UHS. Students
should have their medical insurance card available when visiting
UHS.
UHS Prepaid Plan
This summary is intended to provide
highlights of the plan. A detailed
description of benefits, limitations and exclusions can be found
in the UHS Prepaid Plan brochure, available from the UHS
Managed Care and Student Insurance Office, room 2109, (734) 764-5182.
Eligibility The
Prepaid Plan is available to:
UM students (Ann Arbor campus) who
have been enrolled within the past year but are not taking regular
classes for the current semester
UM alumni (Ann Arbor campus) who have
taken regular classes within the past year
UM students (Flint and Dearborn campuses)
who are currently enrolled or have taken classes within the past
year
Current UHS staff

Basic Review of Coverage:
The UHS Prepaid Plan is administered by University Health Service.
This plan provides basic health care at UHS, similar to coverage
provided to currently enrolled UM students (Ann Arbor campus)
for a specific fee . The Prepaid Plan is sold to affiliated
individuals or to special groups by UHS. Services must be provided
at UHS. Purchase of the Prepaid Plan gives its members access
to medical and specialty clinics within UHS that may not be available
to them under other payment options.

(if purchased after start date) for that coverage period.

Prepaid Plan and Other Insurance:
We strongly recommend that you maintain health insurance coverage
for services not provided under the Prepaid Plan. If you are
currently covered by private insurance, please compare benefits
with the Prepaid Plan. It may be more cost effective for you
to pay for each service you receive (fee-for-service) and seek
reimbursement from your insurance carrier. It is unusual for
insurance companies to reimburse a member for the expense of
the Prepaid Plan because they usually consider it a form of insurance.
