medical. I am 51 and on Medicare for the last four years. Could you please
advise me what type of insurance I should get and who would be able to provide

Medicare Gap Coverage
We have filed suit to stop a disturbing new scam targeted at senior citizens. Insurance agents are visiting seniors at their homes and telling them that unless they provide additional premium payments on the spot, they will lose their Medicare gap coverage. These policies, also known as Medigap coverage, Medicare supplement insurance, or Medicare supplemental insurance, handle expenses not paid by Medicare. The policies reduce out-of-pocket costs, and are essential to the health and peace of mind of countless seniors. What our clients learned later was that they had been tricked into paying thousands of dollars for small-value whole life insurance policies they didn't need. In November 2001 we filed a suit in Naples, Florida against the Pioneer Life Insurance Company, a subsidiary of Conseco Inc., alleging that our clients, senior citizens, fell prey to these empty threats. Names of salesmen involved in these questionable transactions are Jay Steven Goldman, Mark Mittleman, Gregory John O'Connor, Brad Howard Shechtman, Brian Lee Shechtman, and Dean Allen Shechtman (who also goes by the last name Schechtman or Schectman). The importance of having the right coverage
With the rising cost of eldercare, it is critical to make sure that you or an elder has the right level of insurance. Without enough insurance, serious financial problems can occur. That's why it is important to take a close look at insurance policies to determine your level and extent of coverage. Also, look closely at current insurance needs because things may have changed since you or an elder first purchased the original policy. If so, you may need to update the policy.
Medicare/Medicaid/Medi-Gap Insurance
Medicare
Medicare is a federally operated insurance program. Most elders are eligible as long as they are US citizens, have a Social Security number and are 65 years of age or older.
Medicare has two parts-Part A and Part B. Part A covers hospital care for up to 90 days. It also covers care in a nursing home, home healthcare services, and hospice. Part B covers a percentage of care and services provided by a doctor and other healthcare providers, durable medical equipment, outpatient care, and home health services not covered by Part A.
Even if you are covered by Medicare, consider buying a Medi-Gap policy or another type of insurance to cover what Medicare does not. Medi-Gap insurance covers the gap in Medicare insurance.
Medicaid
Medicaid is a federally funded insurance program for many poor and disabled US citizens. To qualify for Medicaid, you must prove that you have few or no assets, and meet the government's criteria for eligibility. However, once you show that you are eligible, Medicaid coverage is far more comprehensive than Medicare. All eligible expenses are covered with a co-payment and many services are included.
Medi-Gap
Private health insurance (Medi-Gap) is designed to cover just what the name
suggests - the gaps in Medicare coverage. Private insurance companies offer this additional insurance to provide the coverage where Medicare falls short. The cost and extent of coverage of Medi-Gap insurance varies considerably according to age, place of care, type of policy and other factors.
Traditional managed care organizations (such as HMOs and PPOs) cover different services and expenses, so you should read the fine print in any policy to make sure it is right for you. Some insurers offer special policies that cover long-term care or nursing home costs. Ways to find private coverage include: asking your employer whether it offers Medi-Gap or long-term care insurance coverage as part of an employee group benefit plan, contacting any professional association or union you may be a member of to see what they offer, and contacting an insurance agency on your own.
Health Maintenance Organization (HMO)
Some HMOs provide all medical services under one roof, while others have a directory of doctors and facilities from which to choose. Medicare-approved HMOs take management responsibility for Medicare from the government. They usually pay for some services in addition to those already covered by Medicare-but may not cover all expenses, such as complete prescription costs.
Preferred Provider Organization (PPO)
In this form of managed care, plan members choose pre-selected healthcare providers within a network, and pay a co-payment (a set fee for certain services). These organizations have arrangements with healthcare providers who have agreed to accept lower fees from the insurance company for their services. PPOs can limit the number of situations in which the PPO will be held liable for payment, which may affect coverage on services such as home care, durable medical equipment, and rehabilitation.
Pros and Cons of Long-Term Care Insurance
Long-Term Care (LTC) insurance is an option to help finance extended care. The need for it can be determined by calculating how much present-day premiums protect against future out-of-pocket costs. It covers mid- to long-term care rather than short-term care. It also covers what Medicare and many private health plans do not. That includes non-medical, day-to-day care at home, and long-term care at nursing homes and other facilities. LTC insurance is expensive but it may be a good value if your particular situation warrants extended coverage.

Medicare
Supplemental Insurance is designed to pick up gaps in Medicare benefits
("Medi-Gap") and is available in 10, now standardized plans, "A-J".
Required disclosure charts provide consumers a very clear picture of the
differences between each of these plans. Private Insurers are not required
to sell each of these model plans but can pick the ones they wish to market.
Prices vary greatly from one company to the next depending on their own
pricing/marketing considerations.
Between
the ages of 64 ½ to 65 ½ Medicare Supplement Plans are available
on a guarantee issue basis (no health questions & no pre-existing condition
limitations). For those who delay going onto to Medicare Part B,
after turning Age 65, their right to guarantee issue starts with their
Part B effective date and ends 6 months after (referred to as "open enrollment").
For all others the plans are medically underwritten. Once you have
a Medi-Gap Plan in California, Law SB 2043 permits a change in plans, also
on a guarantee issue basis, but only to a plan of equal or lesser benefits,
and only within 30 days of any age change.
The
question becomes which of these standardized plans is best suited to your
needs? The answer to this question is often a personal one but many
times centers on ones need, or lack thereof, for Prescription benefits.
Only plans H, I & J offer Rx benefits, and these are limited.
If one doesn't have or foresee any need for Rx then most will consider
only plans A-G. Of these many feel that Plan C is the most cost effective
overall due to the limiting fee charge Law that requires those Doctors
who do accept Medicare assignments (and most do today) to not charge in
excess of the Medicare allowable fees; therefore, plans that charge higher
premiums due to offering coverage for excess charges, as in Standardized
plans F&G/I&J, become less appealing. For many, saving premium
dollars and paying for their own Rx is a better buy based on the theory
that its cheaper to pay the Rx costs yourself and save the premium than
to trade dollars with an Insurer! If prescription coverage is critical
to you another option would be to simply buy a supplemental Rx Discount
Card from a private vendor and/or to set up an investment "side fund" to
pick up any additional gaps (like Rx) should unforeseen needs arise.
After awhile the "side fund" may grow bigger than is ever needed and could
then be drawn down as desired for other income or lifestyle needs although
the "side fund" approach may not work as well for those 75+ due to shortened
life expectancy. We provide investment services through our Broker/Dealer
& Registered Investment Advisory Firm to assist in "side fund" development.
To
evaluate any offering consumers must be provided a Brochure; Outline of
Coverage; Guide to Health Insurance for People with Medicare; Replacement
Form. One company we work with offers a unique feature with its "issue
age pricing" -Vs- attained age pricing. They do not levy rate increases
due solely to ones aging. Most other companies are "attained age"
priced which causes premiums to automatically go up each year as one ages,
or as one moves into older age brackets (i.e. 65-69; 70-74; 75-79;
80-84; 85+). This company markets only the Standardized Plan versions
A, C, F, G, and I.
The
"HMO" forms of Medi-Gap plans are different in that they typically require
no premiums (due to their accepting medicare assignment from the Government
in exchange for a hefty monthly government paid dollar allocation).
Managed care HMO plans additionally require a sacrifice on freedom of doctor
choice since you must receive all benefits only through their contracted
providers.
Some
of the various State "Blue's" (Blue Cross & Blue Shield), for example,
offer their own versions of some of the Standardized Medicare Supplement
Plans. These are referred to in law as "Medicare SELECT" plans and
the major difference is that these plans require the use of specific doctors
& hospitals. The rules are that a carrier must meet the standards
of any "Medicare Standardized Plan" but may exceed it as a means of "customizing"
their own offering.
Seniors
have another very serious need to protect against Long Term Care ("LTC")
Nursing Home and/or Home Care confinements and the costs that accompany
them. For many seniors the annual cost for such care (now averaging -
2001- in California about $45,000 to $50,000 per year, or $125 to $140 per day) could cause them to either completely
loose or seriously deplete assets that would otherwise be used and depended
upon for retirement income. What most do not understand is that Medicare
does NOT cover Long Term Nursing & Home Care and it is not at all likely
that Government will ever be able to provide the resources to cover it.
We have Tax Qualified and Non-Tax Qualified LTC plans, both Private and
State Sponsored, that can help you to effectively address this problem.
We strongly recommend that persons get this type of coverage as soon as
possible.
Remember
that while it's true that money pays for these coverage's only good health
actually buys them; therefore, don't delay your decision to obtain these
types of highly valuable coverage's.

Anaesthetists Warn of Growing Medicare Gap

Anaesthetists
have warned the Medicare gap for their services will widen, following
a doubling in Victoria of insurance costs.
Australian Society of Anaesthetists President, Dr Rod Westhorpe, said
organisations indemnifying Victorian medical practitioners against malpractice
had introduced a new levy to offset projected underfunding of anticipated
liabilities.
The $12000 levy doubles the $12000 cost of insurance for 1999-2000. Dr
Westhorpe cautioned there was no certainty a similar levy would not be
applied in other States and Territories.
He said without urgent action on the part of the Health Insurance Commission
patients would see these costs reflected in the growing gap between Medicare
rebates and actual fees.
"We will be making representation to the Federal Health Minister, Michael
Wooldridge, insisting on his intervention", said Dr Westhorpe.
In the last decade, the average Medicare rebate for an anaesthetic has
risen from $61 to $77.
"But in the same period, the insurance cost per anaesthetic treatment
has risen from $1.50 to more than $20. As well, there has been a raft
of other increases in the cost of running a specialist practice", said
Dr Westhorpe.
He gave assurance that Australia's anaesthetists provided the safest levels
of care in the world, a record that had helped peg insurance costs.
"Patients can rest assured that anaesthetists will continue to provide
the high level of care for which they are renowned, but it's inevitable
fees will rise to adequately cover costs", he said. Dr Westhorpe urged
patients to understand the situation and take issue with the shortcomings
of Medicare.
"The figures show the degree to which we and our patients are subsidising
Medicare. While we're reluctant to add fuel to the growing crisis in Australia's
health system, the situation is that anaesthetists are less and less willing
to continue to subsidise Medicare", he said.

Medicare-Aid
What is Medicare-Aid?
Medicare-Aid is a special Medicaid program for people who have Medicare and
also have limited income and resources. It is a free program that helps pay
your Medicare premiums, co-payments and deductibles. There are several Medicare-Aid
programs which are also known as Medicare Savings Programs: MQB-Q, MQB-B,
MQB-E and MQB-QI.
MQB-Q (Comprehensive Medicare-Aid) covers the Medicare Part B premium,
the Part B deductible for doctor bills, the Part A hospital deductible, 20%
co-payment of Part B approved costs and co-payment for Medicare approved skilled
nursing home care.
MQB-B (Limited Medicare-Aid) covers only the Medicare Part B premium.
MQB-E (Limited Medicare-Aid Capped Enrollment) also covers only the
Medicare Part B premium for those people whose income is too high to qualify
for MQB-B.
MQB-QI (Medicare-Aid Partial Reimbursement) only pays a portion of the
Medicare Part B premium attributed to Medicare Home Health. You only get a check
once a year.

To be eligible for these programs, you must have Medicare and meet income and
resource limits. You may apply for the Medicare Aid programs at your county
Department of Social Services (DSS). You will find them in the phone book under
government agencies. If you are unable to go to Social Services, you may request
a home visit. If you cannot locate the phone number for your local DSS or if
you have further questions regarding Medicaid eligibility after contacting the
local department, call the Office of Citizen Services CARE-LINE Information
and Referral Service toll-free at 1-800-662-7030 (Voice and Spanish) and someone
will assist you. For local calls or calls from outside of North Carolina, dial
(919) 733-4261. The Office of Citizen Services also has a dedicated TTY line
at 1-877-452-2514 or for local TTY or TTY calls from outside of North Carolina,
dial (919) 733-4851 for deaf and hearing impaired.
The Office of Citizen Services can also provide you with information and referrals
on other Department of Health and Human Services programs along with other government
and non-profit agencies that may be helpful, as well as tell you about prescription
assistance programs available through drug manufacturers. (Office of Citizen

How long can I receive benefits after I qualify?
Once an application is approved, you receive benefits for 12 months. Then you
must complete a review of your eligibility each 12 months to continue receiving
Medicare-Aid. A worker at the department of social services will contact you
when it is time for your review. You must continue to meet the income and resource
guidelines.
Can I have Medicare Aid if I have private Medi-gap insurance?
Yes, you can apply for Medicare Aid if you have private Medi-gap insurance.
However, you cannot receive both benefits at the same time. If you are approved
for Medicare Aid, your Medi-gap insurance will be suspended for up to 24 months.
This means you can resume your Medi-gap insurance policy during that 24 months
if your Medicare Aid benefits stop. After 24 months if your Medicare-Aid benefits
stop, you must reapply for Medi-gap insurance. You are not guaranteed to be
approved.
Will Medicare Aid pay for my prescriptions?
No. Medicare Aid only pays your Medicare premiums, co-payments and deductibles.
What happens if I have to go into a nursing home?
If you need help with the cost of care in a nursing home, you must apply at
your local DSS. MQB-Q covers only the Medicare co-payment for Medicare
approved skilled nursing home care. After the first 20 days, nursing
home care is not covered by Medicare Aid.
